New Patient Information Forms Package
New Patient Forms PDF File contains all the forms you will need to become a patient here at Health Centered Dentistry. Please fill out these forms, and mail them back to us (we need the original signatures). We will review the information to determine which type of new patient initial exam would best suit your situation. We will then send you a letter letting you know we have received your paperwork and indicating what type of New Patient Initial Exam we believe will best suit your situation. Once you receive that letter from us and have had a chance to look it over, please call us to schedule your visit.
